Family discount: Families of three or more enrolled members receive a 10% discount on membership fees for every family member.

X-rays include the radiologist’s reading fee. Lab and procedure charges depend on the service. Payment is due at the time of your Urgent Care visit.

Family Practice Physician
Physician Associate
For Urgent Care, select “Heartland Medicine – URGENT CARE” in the Branch location field. The form includes membership questions, but completing it for an Urgent Care visit does not enroll you in a membership. For membership, read the patient contract and select “Heartland Medicine – Members.”
Urgent Care addresses an immediate illness or injury. It does not establish the clinic as your primary care provider. Routine prescription refills, continuing referral needs and ongoing medical management should be coordinated with your primary care provider.
We do not accept or bill insurance of any kind. Patients pay the clinic directly. Direct Care membership is not health insurance.
